As part of Project TEACH, C.A.P.E.S. is very proud of the linkage and referral system that we offer to all PCPs. We help build relationships between PCPs and community mental health providers and serve as the bridge that links behavioral health and primary care services for children and families.

To date, we have linked with over 350 mental health practitioners and obtained detailed information about their practices. With this information we have created a database that we use to help PCPs obtain patient-specific referrals.

By sharing just 4 pieces of information: a Patient’s Age, City of Residence, Diagnosis or Type of Therapy Needed, and Insurance Carrier, we can quickly provide referrals to therapists within the child’s community. By taking the “guess-work” out of the referral process we are helping PCPs better assist their patients.

If you would like a referral for a child or adolescent in your practice, then please use this service. There are three different ways to request this information.

If you would like your practice information added to our database and shared with PCPs, then please complete the Mental Health Provider Form, and fax or mail it to us.
